Important Clinical Disclaimer: Payment for an appointment does not guarantee that any specific medication, including a controlled medication, will be prescribed. All treatment and prescribing decisions are based on the provider's clinical assessment, medical appropriateness, and applicable federal and state prescribing requirements.
: For follow-up appointments requiring additional time due to multiple concerns, significant medication changes, complex treatment planning, or other clinically appropriate needs.
Appointment & Cancellation Policy: Patients are provided a 15-minute grace period for scheduled appointments. Patients who are more than 15 minutes late may need to reschedule their appointment. A $75 missed-appointment fee applies to missed appointments.
